Forms For Proposals (Back to Top)

Several sponsors, e.g., Public Health Service, American Cancer Society, National Science Foundation, provide application kits (forms and instructions) for proposals. Others, e.g., Air Force and NASA simply provide a guide to be followed in proposal preparation. Some Federal sponsors such as the Departments of Education and National Oceanic and Atmospheric Administration (NOAA), and the Small Business Administration, use the standard SF-424 Federal Grant-in-Aid Form. These forms and guides may be requested directly from the sponsor, or may be available from the Office of Sponsored Programs. (See also the various Web sites of federal agencies providing standard forms on-line.) In those instances where neither an application kit nor a guide is provided, Sponsored Programs can help you shape the proposal.

Appendix I provides a checklist of items that should generally be considered in the preparation of proposals.

 

Cover Page (Back to Top)

Appendix II provides an example of a cover page for your proposal when the sponsor does not specify some other format. This page may be used for either Federal or foundation sponsors.
